Job Description

POSITION SUMMARY:  Under the direct supervision of the Director of Nursing, will provide resident focused programs, services, and/or interventions to residents ofPioneer Ridge.

 

MAJOR RESPONSIBILITIES:

  1. Provides assessments, interventions and counselling services to residents and families of the Pioneer Ridge.
  2. Provides counselling to individual residents and families who require crisis supportive intervention of a short term basis.
  3. Attends multidisciplinary admission and annual care conferences, supporting a holistic approach to resident care.
  4. Acts as a resource in developing care plans for residents.
  5. Documents in accordance with Pioneer Ridge health care records policies.
  6. Acts as a resource to family support groups to assist them in coping with the admission of their family member.
  7. Maintains a current knowledge of relevant legislation and senior’s issues within the division, the department and community.
  8. Participate in Home Total Quality Management Program.
  9. Performs related work as assigned.

 

QUALIFICATIONS:  

 

Education/Experience:

  • A college social service worker diploma from a recognized educational institution; and
  • Two years of experience working with the elderly

 

Skills/Abilities:

  • Excellent oral and written communication skills
  • Effective interpersonal skills are required
  • Must be competent within the meaning of the Ontario Occupational Health & Safety Act

 

CONDITIONS OF EMPLOYMENT:

  • Must have and maintain membership in the Ontario College of Social Workers and Social Service Workers
  • Must undergo a successful police records check, Type 2
  • Must be able to work occasional evenings and weekends as required
  • Must have successfully undergone a "two step" tuberculosis skin test within the 12 months prior to employment
